Oversight your team can act on.

Who this is for

When you are responsible for the quality of care.

For organizations that purchase, commission, or oversee healthcare services and need a clearer view of performance.

  • Counties & government agencies
  • Correctional agencies & oversight bodies
  • Public health departments
  • Healthcare organizations & NGOs

A focused assessment or ongoing review: the engagement defines the questions, measures, access to records, and reporting cadence.

What we monitor

From contract requirements to the care people receive.

We connect reported performance with the records and clinical context behind it. Review areas are selected for your program and agreed scope.

  1. Vendor & contract performance

Review service obligations, performance measures, and supporting records against the agreed requirements.

  2. Staffing & coverage

Compare planned coverage, reported hours, and available evidence of staffing and service delivery.

  3. Access & continuity of care

Examine requests for care, timeliness, referrals, follow-up, and transitions between services or settings.

  4. Clinical records & medications

Review sampled documentation and medication continuity in the context of the clinical question being assessed.

  5. Data & reporting accuracy

Check measure definitions, completeness, source records, and inconsistencies that can affect reported results.

  6. Corrective-action follow-up

Track the response to findings and review evidence supporting progress or completion.

How monitoring works

Define. Review. Validate. Follow through.

Clear review criteria and traceable findings help leadership understand both the evidence and its limits.

  1. 01

    Define the review

Agree on requirements, measures, reporting periods, records, sampling, responsibilities, and escalation procedures.

  2. 02

    Examine the evidence

Review authorized clinical records, staffing information, operational data, policies, and vendor reports relevant to the scope.

  3. 03

    Validate the findings

Reconcile reported results with source information. Identify discrepancies, missing records, and limitations before drawing conclusions.

  4. 04

    Report & revisit

Communicate findings, recommended next steps, and open questions. Review the response and supporting evidence in subsequent cycles.

Accreditation Readiness & Standards Alignment

PetraMED helps healthcare programs assess readiness, organize evidence, and track improvements before accreditation reviews and between review cycles.

For correctional healthcare, support can be scoped around relevant National Commission on Correctional Health Care (NCCHC) and American Correctional Association (ACA) healthcare standards, based on the facility type and accreditation program.

PetraMED provides readiness and advisory support. Accreditation decisions are made by the applicable accrediting organization.

Clear responsibilities

Monitoring with a defined role.

In a monitoring engagement, PetraMED evaluates performance, communicates findings, and recommends next steps. Designated clinical and operational leaders review and implement changes.

The engagement defines review authority, access to information, escalation, and follow-up. Monitoring responsibilities are distinct from any separately contracted medical direction, staffing, or program management services.

Common questions

Before we begin.

Scope and expectations should be clear from the first conversation.

What is healthcare monitoring?

Healthcare monitoring is a structured review of service delivery, clinical information, and reported performance against defined requirements. It identifies gaps and follows the response through reporting and review.

Who hires PetraMED for monitoring?

Monitoring is relevant to government agencies, correctional agencies and oversight bodies, public health departments, healthcare organizations, and NGOs responsible for healthcare services. The scope depends on the program, setting, and oversight need.

Can PetraMED help with accreditation readiness?

Yes. Support can include readiness assessments, policy review, evidence organization, and corrective-action follow-up. For correctional healthcare, the engagement can address applicable NCCHC and ACA healthcare standards. The facility type, accreditation program, current standards, and review scope are established at the outset. The accrediting organization makes the accreditation decision.

What records are needed?

Depending on the scope, records may include contracts, policies, staffing schedules and reported hours, service and referral logs, performance reports, and authorized clinical records. Access arrangements and handling requirements are established before review. Do not submit patient records through this website.

How are vendor reports checked?

Reported measures are reviewed against their definitions, reporting periods, and available source information. Reconciliation and sampled record review can identify inconsistencies, incomplete data, or unsupported conclusions. Findings should state the evidence and its limitations.

Is monitoring a one-time review or ongoing?

An engagement may focus on a defined assessment or recurring monitoring. Reporting frequency, follow-up, and any on-site or remote work are established in the engagement scope.

Does the monitoring team operate the healthcare service?

Monitoring evaluates performance and tracks the response to findings. Clinical and operational leaders retain their assigned responsibilities for service delivery and implementation. Any separate PetraMED leadership or management services require a defined scope.
